The short answer
A 4 inch (100 mm) slab can hold a small hot tub if it's sound, reinforced and sitting on a compacted base. A thin, cracked or unreinforced slab is a risk under a full tub, so check the slab and the tub maker's base requirements before you plan around it.

Can a 4 inch concrete slab hold a hot tub?
Often, yes, for a small or mid-size tub. As general practice, 100 mm (4 in) is a common minimum for a hot tub pad. The thickness on its own doesn't settle it. What's under the slab, whether it has steel in it and what shape it's in matter as much.
A full tub with people in it puts a lot of weight on a small area, and it sits there year-round. A slab that was fine as a patio can crack or settle once that load arrives. Larger tubs and swim spas usually need a thicker slab with more steel, and the tub maker may set a higher minimum.
How do you tell if your patio will hold it?
Look for cracks that have opened up or have one side higher than the other, corners that have sunk, and puddles that sit on the surface. Any of those says the base under the slab has moved, and a hot tub will make it worse.
Find out how thick it is and whether it has steel in it. Drilling a small test hole or looking at an exposed edge shows the thickness. Whoever poured it, or the house plans, may tell you about steel. If you can't find out, treat it as unreinforced.
Check that it's level enough. A tub needs a flat, level surface so the water line sits even. A patio sloped to drain away from the house may be fine for chairs and wrong for a tub.

Do you need to put a hot tub on a concrete pad?
No, but you need something firm, level and fully supporting under it. A concrete pad is the most solid option. A compacted gravel pad with a solid edge, or interlocking spa pads on gravel, can work for some tubs. The tub maker's guide says which it accepts.
Bare grass or soil won't do. It settles unevenly and stays wet, and the tub's frame twists as it moves.
What size concrete pad do I need for my hot tub?
At least the size of the tub's footprint. Most people go bigger so there's room for steps, a cover lifter and to get at the access panel for service. The tub's spec sheet gives the footprint and shows which side the panel is on.
Think about where water goes too. The pad is poured level, so the ground around it should fall away from the pad and the house.

Does a hot tub need an electrical permit in BC?
Yes. Technical Safety BC says installing a hot tub needs an electrical permit. For a new pad, plan the conduit route before the pour so it runs under or beside the slab cleanly.
In Maple Ridge, the building bylaw needs a pool permit for hot tubs “except for self-contained hot tubs with a locking cover”, and counts as a pool anything that holds water deeper than 0.45 m or with a surface area over 14 m². A big swim spa should be checked with the City before you order it.

Reuse the patio or pour a new pad?
If the slab is sound, level, thick enough and reinforced, and the tub maker is happy with it, reusing it saves a pour. If it's cracked, sunk, thin or you can't tell what's in it, a new pad is the safer call. That might mean cutting out part of the patio or pouring a pad beside it.
Send a photo of the patio and the tub's spec sheet with the booking form on this site. John will tell you whether the slab will carry it or what a new pad needs.

The bottom line
A 4 inch (100 mm) slab can hold a small hot tub if it's sound, reinforced and sitting on a compacted base. A thin, cracked or unreinforced slab is a risk under a full tub, so check the slab and the tub maker's base requirements before you plan around it.
More questions
Can a hot tub sit directly on the ground?
No. Bare ground settles and stays wet, so the tub needs a compacted gravel base at the least, with a pad or slab on top as the tub maker requires.
How long does a concrete pad need to cure before a hot tub?
Concrete is commonly taken to reach its design strength at around 28 days. When the tub can go on depends on the mix, the weather and the tub maker's guide.
Why do you put a tennis ball in a hot tub?
The fuzz soaks up some of the oils from skin and lotion in the water. It helps a little but doesn't replace the filter.
What is the best thing to put under a hot tub?
A reinforced concrete pad on compacted gravel is the most solid long-term base. Gravel or spa pads can suit some tubs if the tub maker allows them.
